Sport Health Insurance Waiver Form

As the parent or legal guardian of a minor participant in the Team Eritrea Festival 2025 sports activities in Oakland, California, I acknowledge that health insurance coverage is required for all participants.

 

I understand and agree that I am choosing to waive this requirement because my child or children are already covered under an existing health insurance policy. By proceeding with participation in the sports events, I voluntarily assume full responsibility for any and all medical expenses that may arise in connection with my child’s participation.
 

 

Furthermore, I release and hold harmless the organizers of the Team Eritrea Festival 2025, including its affiliates, volunteers, and associated personnel, from any liability or obligation related to the provision or verification of health insurance coverage.
 

By agreeing to this waiver, I confirm that I have read and understood the above statement and accept these terms willingly and without reservation.
